Home Health ServicesHome healthcare services can encompass a wide range of programs and assistance - from in-home-nursing and rehabilitation after hospitalization to ongoing daily living assistance.

The Gardner VNA is here to assist you and your loved ones with everything from rehabilitative services such as occupational and physical therapies to everyday activities such as personal hygiene, meal preparation, shopping, and periodic check-ins. 

Program Components and Services

Our Home Healthcare Services program includes:

  • RN Community Health Nurses providing skilled nursing services and coordination of the care plan ordered by the attending physician.
  • Certified Home Health Aides assisting with the hands-on personal care needed to maintain the patient's health.
  • Rehabilitative Therapists providing exercises and activities designed to restore physical function. Therapists evaluate the need for equipment to achieve and maintain independent function. Therapies include occupational, physical & speech.
  • Telehealth System assisting with disease management right in a patient's home. This system collects vital signs, as well as symptom information that is transmitted directly to the Gardner VNA via the phone line.
  • Medical Social Workers offering practical counseling for patients and their families concerning emotional, economic, and other problems related to their illness. They also refer clients to other specialized services and assist in long-term planning.
  • Wound, Ostomy & Continence Nurse offering specialized nursing skills to individuals with wounds or ostomies.
  • Maternal Child Healthcare offering nursing care to sick children and newborns and education for new moms.

Referrals

Referrals can be made by physicians, case managers, discharge planners, hospitals, clergy, friends, family or self-referral. A nurse will visit to assess eligibility at no charge.

Please Note: Fees can be adjusted on a sliding scale based on the client's ability to pay.
