My Home Connection

We believe that older, chronically ill, and disabled adults should be able to remain independent and maintain the highest quality of life possible while preserving their life savings.  My Home ConnectionTM is a unique, private duty service of the Gardner VNA that enables clients to stay at home, providing comprehensive support to them and their families or caregivers.

How it works

My Home ConnectionTM services begin with a complete assessment of the client and his or her needs. Every client receives the assistance of a professional care manager who develops a plan of care that blends medical, financial, legal, mental health, insurance and psychosocial requirements based on the client's individual needs and choices. The professional care manager works on the behalf of the client and his or her family, acting as an advocate for them when necessary while helping them set goals and evaluate options.

All of our My Home ConnectionTM staff are trained and experienced professionals who have undergone a rigorous and comprehensive background check for the safety and well-being of our clients.  All caregivers of the program receive ongoing supervision by a registered nurse and attend trainings annually and as needed.  As employees of the Gardner VNA, Worker's Compensation and applicable taxes are paid by the agency for the employee, relieving you of that responsibility.

Safety Analysis:

  • Home accessibility assessment, design, and modification referrals
  • Information regarding housing alternatives
  • Availability of Lifeline personal emergency response and medication management systems through the Gardner VNA

Financial & Legal Referrals:

  • Specialized financial planning referrals for asset protection
  • Elder Law referrals
  • Estate planning

Insurance Assistance:

  • Medicaid applications
  • Social Security Disability Applications
  • Information, assistance and advocacy with insurance eligibility
  • Long-term care insurance referrals

Service Area:

  • North Worcester County and Franklin County
    (Other areas are eligible for our services, just call for more information)

Referrals:

Referrals can be made by anyone interested in the My Home Connection program, including people in need of the service themselves.
