Telehealth: A disease management system to help patients stay in their own homes

Providing peace of mind to patients and their families every day.

Telehealth is a disease management system we use to monitor at-home patients' vital signs and symptom changes to help prevent an illness from getting worse. Using a small device called a Health Buddy, this advanced monitoring system collects patient data daily and transmits it to our office where it is reviewed by one of our clinicians. This state-of-the art system makes patient monitoring more efficient and convenient for both the patient and our nurses, enabling us to improve patient outcomes and reduce re-hospitalizations.

In addition to helping us monitor our patients and provide faster, better care, Health Buddy also helps us keep our patients informed about their illness and receive personalized instructions through an easy-to-use, easy-to-read screen and simple control features. The Telehealth system is designed for patients who experience frequent ups and downs in their condition, including the following:

  • Diabetes
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Heart Failure
  • High Blood Pressure
  • Coronary Artery Disease

Our Telehealth monitoring system enables us to provide critical healthcare data quickly to both  patients and their healthcare providers, enabling better, faster, and more informed care to help prevent medical crises and improve the patient's well-being.
