Assisted Living Program for the Elderly and Disabled

The Home Assisted Living Program at the Gardner VNA is designed to restore and maintain physical and emotional well-being.  It promotes independence in order to prevent or postpone long-term placement or alternative living arrangements for qualified persons living in elderly or handicapped housing.  This goal is met by providing a comprehensive service plan, which includes the collaboration of Nursing, Medical Social Work and Personal Care Assistant (PCA) services in conjunction with other community based programs as needed. The program services areas in both Worcester County and Franklin County.

This is the Group Adult Foster Care Program offered through MassHealth.
