Home Care Services
The GVNA is a multifaceted organization, serving North Central Massachusetts and Franklin County, providing a broad array of home health services. Our clients include all age groups from infants to the elderly.
Since our inception in 1906 by a group of local nurses and physicians, our agency has grown dramatically, but continues to uphold the ideals and goals of our founders. Today we provide thousands of visits annually, while remaining true to our belief that people deserve the highest quality of skilled and compassionate health services provided in the comfort and security of their own homes.
Program Components/Services:
The Home Health Services program at the GVNA includes the following disciplines:
Community Health Nurses provide skilled nursing services and coordinate the plan of care ordered by the attending physician. This would include LPN's and RN's.
Certified Home Health Aides assist with the hands-on personal care needed to maintain the patient's health.
Rehabilitative Therapists provide exercises and activities designed to restore physical function. Therapists evaluate the need for equipment to achieve and maintain independent function.
Medical Social Workers offer practical counseling for patients and their families concerning emotional, economic, and other problems related to their illness. They also refer clients to other specialized services and assist in long-term planning.
Wound, Ostomy & Continence Nurse offers specialized nursing skills to individuals with wounds or ostomies.
Maternal Child Health offers nursing care to sick children and newborns and education for new moms.
- The Gardner VNA is pleased to provide Heywood Hospital Maternity patients a complimentary nursing visit after the delivery of a child. The first few days home with a new baby is an exciting time, but often questions about the care of a baby arise. We understand your concerns and are happy to answer any questions you may have when we make the complimentary visit to your home. The Gardner VNA is able to offer this service through grant funds received from the United Way.

Ashburnham, Athol, Barre, Erving, Fitchburg, Gardner, Hubbardston, Leominster, New Salem, Orange, Petersham, Phillipston, Royalston, Templeton, Warwick, Wendell, Westminster and Winchendon. For more information about additional towns, please give us a call.
Referrals:
Referrals can be made by physicians, case managers, discharge planners, hospitals, clergy, friends, family or self-referral. A nursing visit to assess eligibility is available at no charge.
