My Home Connection is a unique service of Gardner Visiting Nursing Association, Inc. that supports and allows older, chronically ill and disabled adults and their families to remain independent and maintain the highest quality of life while preserving life savings. My Home Connection serves clients in Worcester County and Franklin County.
Program components:
Services begin with an assessment by a professional care manager.
The care manager develops a plan of care based on a client's individual
needs and choices. This plan looks to blend medical, financial,
legal, mental health, insurance and psychosocial needs. We provide
advocacy and help clients and their families evaluate options and obtain
goals.
For the safety and well-being of our clients, a comprehensive
and complete background check in done on all employees. All
caregivers of the program receive ongoing supervision by a registered
nurse and attend trainings annually and as needed. As employees of
the Gardner VNA, Workmen's Compensation and applicable taxes are paid by
the agency for the employee, therefore relieving you of that
responsibility.

- Information, assistance and advocacy with insurance eligibility
- Long-term care insurance referrals

Referrals can be made by anyone interested in the My Home Connection program or concerned about themselves or a loved one in need of our services.
