The Gardner VNA offers patients the convenience of monitoring
health conditions in the home.
The Gardner VNA is excited to move into this state-of-the-art
monitoring in an effort to reduce re-hospitalizations and improve
patient outcomes.
Telehealth is a remote disease management system that monitors
vital signs and symptom changes to help prevent an exacerbation of an
illness. This information is
transmitted directly to the Gardner VNA
to
be reviewed by a clinician.
It uses an appliance, called a Health Buddy, to collect data on a daily
basis.
The “Health Buddy” will allow the Gardner VNA Telehealth nurse to
monitor patients via the computer.
Our service area includes 24 cities and towns in
Worcester County and the North Quabbin area.
The Gardner VNA having the ability to monitor patients right in
the office will have an immeasurable benefit to our patients.
The patient benefits include personalized instruction,
reinforcement of information, and encouragement; therefore, empowering
the patient to improve their own care.
The system is designed for patients who experience frequent ups
and downs in their condition.
Types of conditions that the Gardner VNA will be monitoring
through this system include:
The benefits of this program from a medical point of view are
numerous, the Gardner VNA will be able to deliver vital sign data,
trending and graphs to physicians very quickly in order to prevent any
medical crisis. This will
also provide Gardner VNA nurses and patients with instant access to this
same data. Patients will be notified if their results are out of the
norm so they can contact there physician immediately for further
instruction.
